1. Background/Overview
During your work placement, you will be undertaking a variety of duties and tasks that relate to your course and the role of a leisure and health worker. You will need to successfully demonstrate that you can perform tasks required in the work environment and collect evidence of this competence.
Your workplace assessments are divided into three sections (the three portfolios) and these need to be submitted in three parts, following the Work Placement Training Plan agreed upon with Open Colleges. This Portfolio is the second section and you’ll need to submit it once it’s fully completed.
Your Workplace Supervisor will give you guidance and feedback on your performance, supporting you to complete the tasks in an appropriate and professional manner.
Evidence of your performance must be collected and submitted to your Open Colleges Trainer/Assessor.
2. Work Placement Portfolio
Your Open Colleges Trainer/Assessor will talk to your Workplace Supervisor (WPS) about all the tasks in each activity. Your WPS will confirm that you have completed each task according to the task instructions, and so you need to ensure your WPS witnesses the individual tasks as you complete them.
This portfolio needs to be uploaded at the end of your placement as a single Word document. Make sure you have completed all the tasks, including signed supervisor section and the video recording, and upload your work to OpenSpace.
Note to students: Client confidentiality
You will need to have a conversation with your WPS in relation to confidentiality as you will need to take copies of clients’ individual plans. Ensure that all client personal details are blacked out.
Selected clients or their legal guardian will need to sign the ‘Consent Form’ (available in Appendix 12) before commencing accessing their personal information or implementing any activities.

Before commencing with the video recordings, you will need to ask participants’ permission for recording and complete the ‘Video Recording Consent Form’ in Appendix 13 with all participants’ name and signature (including yours), then upload to OpenSpace together with the videos and portfolio.
Handy tips for recording and uploading your video to OpenSpace:
? You can create your video using a mobile device or laptop. You can set up the device yourself or ask a friend or colleague to help with the video recording. Before you start recording your video, please look at the OpenSpace video function.
? Before starting your video, write down/think through a plan on what are you going to demonstrate or discuss with other participants. You may want to draft a short script for the participants and for yourself as a guidance on what are you/them going to say in the video.
? As part of your video recording you must include an introductory statement stating your name, student ID, qualification and assessment number.
? Speak clearly and in a professional manner. Make sure you and any other participants (if any) are clearly visible and well lit.
? Do not press ‘Submit’ until you have included all videos, consent forms and the completed portfolio. If you press submit, it will alert the assessor to mark your assessment. Description
Contribute to the creation, planning, coordination, delivery and evaluation of appropriate leisure and health activities for individual client(s) and client group.
In consultation with your supervisor, you will be working on three leisure and health activities for your client or client group with the aim of documenting four activity modifications and or adaptions that are investigated, including a visit to a suitable outing venue.

Task 1: Identify activities to match leisure and health needs of the client or group
Revisit the three lifestyle plans which were developed for the selected clients or client group referred to in Portfolio 1.
Work with the clients or client group and contribute to identifying activities – you may want to refer to the Structured and Unstructured Activities in Appendix 1 to match leisure and health needs of the client or group of clients.

Task 1: Self-evaluation
Conduct a self-evaluation using the Reflection Diary (Appendix 8) and Reflection template (Appendix 9).
Think about the activities you completed in Portfolios 1 and 2 and reflect on your own skills and practices. Meet with your supervisor and/or peers and undertake a self-evaluation on how you are progressing with the allocated tasks.
Record a 3-5 minute video of the professional conversation regarding your strengths and areas for improvement with your supervisor or peers, discussing their feedback and ideas for you to improve in the identified areas.
Make sure that the ‘Video Recording Consent Form’ is completed and signed by all participants. See Open Colleges Assessor’s report for criteria you will be assessed against.
As part of your video recording you must include an introductory statement stating your name, student ID, qualification and assessment number.
To successfully complete your self-evaluation, you will need to:
a. Reflect on and recognise the effect of values, beliefs and behaviour in your work.
b. Conduct a two-way, open and evaluative feedback conversation with co-workers or peers.
c. Actively seek and reflect on feedback from clients, organisations or other relevant sources.
d. Complete your self-evaluation and reflective tasks using the templates in Appendix 8 and Appendix 9.

To successfully complete your Development Plan, apply the following aspects.
a. Identify potential support networks – both internal and external to the organisation.
b. Identify and seek specialist advice or any further training.
c. Identify and understand the requirements of self-care and additional support.
d. Complete your Development Plan using the template in Appendix 10.

As it is important to follow through with your development, you should always have your Development Plan at hand as it is a living document in which, as you develop or improve a skill, you can update your progress.
At the end of Portfolio 3, schedule a meeting with your WPSand review your Development Plan to identify areas where you have improved your skills as a leisure and health worker.
