Submission: Online via Turnitin
Format: One file only, saved in Microsoft Word.
Length: 800 words

Description: For this task, students will be asked to critically examine a real-life patient safety failing. Students will conduct an examination of a patient safety failing to identify important insights into key elements of patient safety, identify lessons learnt and to provide recommendations for system improvements and present their findings in a ’brief’ of the event. Further students should provide recommendations as to quality and safety principles so far in the subject could be applied to prevent similar incidents in future practice. Criteria:
- Clear and concise summary of the quality and safety failing with nil inconsistencies and identifying all key points.
- Clear and concise summary of recommendations as to how the principles of quality and safety learnt so far in the subject could be applied to prevent similar incidents in the future
- Use of academic tone and language to convey message in a professional and concise manner including proper use of spelling and grammar
